Interested candidates are requested to submit the following 4 documents for the Africa CDC’s consideration:

1. Curriculum Vitae of the Consultant(s)

2. Technical Proposal not exceeding 3 pages on:

• Understanding and interpretation of the TOR
• Methodology to be used in undertaking the assignment
• Time and activity schedule

3. Personal Capacity Statement with:

• Relevant experience related to the assignment (include samples of two most recent similar works and/or references for the same)
• Contacts of at least 3 organisations previously worked for

4. A financial proposal not exceeding 1 page:

• Consultant's daily rate in US$
• Other costs e.g., travel
• Total cost
